DCHS Star Award Nomination Form

DCHS Star Awards are designed to recognize an individual whose action was significant to another employee. Awards will be given out once a month with a maximum of five winners, each receiving a $10 gift card. If there are more than five nominees, those who do not win will be put back in the pool for next month's drawing. A copy of this form will be sent to your email address.
