DCHS Superstar Awards are designed to recognize an individual or group whose action was outside of normal work expectations and has had a significant impact on internal or external DCHS customers or departmental operations. Individual Super Star nominees recieve a $25 gift card and are entered into a drawing for $250. Group Super Star nominees are entered as a group into a drawing for $500. All nominees will be recognized in Connect, the DCHS employee newsletter, and invited to an annual pizza party to celebrate their nominations. A copy of this form will be sent to your email address.
